Columbine Commons Health & Rehab is located at 1475 Main St. in Windsor, CO. This beautiful 30-bed facility offers long term and rehabilitation services. Within walking distance is our newest building; it is another 30-bed state of the art facility just waiting to open!

Compensation

  • Starting wage $100-$115k/yr. depending on experience

Work Schedule

  • Full-time
  • Monday through Friday

Responsibilities include

  • Plans, directs, coordinates, and evaluates the overall operations of the facility to ensure the highest standard of care is given
  • Administers and supervises all aspects of the operation of the facility in accordance with policies set by the governing board
  • Implements policies, objectives, and philosophies regarding resident care
  • Develops and implements preparation of all required records to assure economic operation of the facility
  • Facilitates compliance with all applicable State and Federal Regulations governing skilled nursing facilities
  • Oversees and assists department managers/supervisors procedures and policies related to staffing/staff development in each department
  • Determines the care of residents is maintained at the highest professional level, meets all standards prescribed by regulatory agencies and the governing board
  • Develops and implements, with the assistance of all department heads, standards for employee conduct while on duty, working hours and rates of pay
  • Promotes patient/resident's right to fair treatment, individuality, privacy, property, civil rights and the right to lodge a complaint
  • Provides efficient communication to departments regarding admissions, discharges, room holds, level of care, and room/unit changes
  • Coordinates Quality Improvement Committee meetings and partner with department managers for the Quality Assurance of each department for ongoing assessment of medical and psychosocial needs of residents
  • Ensures processes are in place to conduct resident care conferences and appropriately reviewed
  • Evaluates and implements annual facility/departmental budgets
  • Oversees policy for food and operational supply purchases
  • Participates with management for selection of vendors and contracts for services
  • Ensures HIPAA regulations are followed

Education and Experience Requirements

Bachelor's degree from a four-year college or university, five years or more of work experience in sub-acute or rehab facility, or equivalent combination of education and experience.

Must fulfill Administrator license requirements in the State of Colorado and hold a current license.
